We provided care to patients receiving funded care in the community. I was responsible for a team of health care workers-my role included but was not limited to meeting clients and going over care plan, notifying of expectations of the staff coming into their homes. Reporting any changes in client's condition-during home visits or as reported by care staff after investigating report. Speaking with case managers, physiotherapists, and any other discipline involved in client's care to ensure most appropriate plan of action with continued services being provided. On site visits to ensure care plan being followed. Ongoing evaluation of care staff and provided any additional support such as training for tasks i/e changing catheter bag, donning compression stockings, application of topical ointments, administration of medications from blister packs, using various in home lifts-floor lifts/ceiling lifts for partial-fully non weight bearing clients. Conflict resolution between client/family and care staff as well as between care staff.
